REFERRAL FORM

If your healthcare provider suggests you have some imaging (i.e. X-Ray, CT, Ultrasound, MRI, etc.) undertaken they will provide you with a Referral Form, outlining the type of imaging required. You cannot have imaging undertaken without this Referral Form.

CHOOSE YOUR PROVIDER

This Referral Form may nominate a particular radiology provider. However, you are free to take this Form to any radiology provider. The “nominated” provider may charge you a fee for the service, whereas at IIP that service may not involve any out-of-pocket costs. IIP is independent and aligned only with Peninsula Health. We are there for you – the choice is yours!

Should you decide that IIP & Peninsula Health are your best option, please contact us on (03) 9788 4501 to arrange a suitable time for your imaging to be undertaken. At the time of booking you may be given some pre-imaging instructions. These instructions are to ensure the imaging produces the best results, thereby assisting your healthcare provider in determining the most suitable course of treatment. Please ensure that you undertake any pre-imaging instructions.
